POSITION TITLE: OUTMemphis Case Manager

REPORTS TO: Director of Case Management

STATUS: Full Time, Exempt 

SALARY Range: $50,000-56,000 plus health and 401K benefits and professional development opportunities.



SUMMARY: 
This OUTMemphis Case Manager will support the sustainability of the OUTMemphis mission to provide quality case management services to the LGBTQ+ community of the Mid-South. This licensed Case Manager position reports to the Director of Case Management and consults with the Senior StaR Consultant. The OUTMemphis Case Manager will build and maintain relationships with local mental health agencies, inclusive of creating and sustaining a referral bank of culturally competent and LGBTQ+ trained therapists to meet the diverse needs of the population we serve.

This position provides case management, referrals, and supportive services for participants enrolled in case management services. The OUTMemphis Case Manager will support LGBTQ+ individuals experiencing homelessness. This position will collaborate with the OUTMemphis Case Management team to track data and provide programs and services, in compliance with regulations, grant requirements, best practices, and in line with our mission and values.

This role includes the development of allies and partnerships for the LGBTQ+ community to enhance the delivery of case management services. The OUTMemphis Case Manager must provide OUTMemphis Community Center and YEC coverage as needed, greet visitors and work with program teams to ensure that the quality of programs and services are maintained.

DUTIES AND RESPONSIBILITIES: 

    • Provide case management and general support to LGBTQ+ adults, Transgender individuals and individuals living with HIV; inclusive of screening and assessing clients i.e. intakes; creating, implementing and monitoring individual service plans; crisis intervention; connecting clients to health, mental health, employment, housing and other community resources
    • Provide crisis intervention, referrals for short term mental health services and psychosocial case management as needed and appropriate.
    •  Provide home visitation, transportation support, and services on-site (at YEC, CC, and Donation Center).
    •  Maintain a case load of 20 to 40 clients monthly for case management needs (i.e. the case load will start with 20 and build slowly).
    • Prepares and implements case coordination plans by conducting assessments for clients referred, assessing needs and developing case management plans to assist clients and families in obtaining case management services and gender affirming care and therapy referrals
    • Collaborate with the YEC and Community Center staff and volunteers on the day-to-day operations of the YEC and Community Center.
    • Obtain HIV Heat training to provide support to the health and wellness staff when applicable
    • Assist in outreach and education for communities at highest risk for comorbid health concerns related to HIV, including gay and bisexual men of color and transgender women of color.
    • Collaborate with our communications team to create social media and marketing materials to promote case management services.
    • Manage related programs budget including the input of receipts for documentation of program related expenses.
    • Maintain accurate, current, andand confidential records for case management participants in conjunction with HMIS and local and federal laws and regulations.
    • Collaborate with the Director of Case Management, Chief of Programs Officer and Health & Wellness Director to provide mental health referrals and case management services
    • Provide data entry and case management for all sexual health and health and wellness initiatives, especially HIV testing, linkage to care, Ryan White, and PrEP/PEP navigation.
    • Demonstrates a strong ethical perspective while providing resource-specific information to clients and families, initiating contact with appropriate resources for referrals (ie. medical, therapeutic, social service programs, etc.)
    • Participate in “All Staff” duties, including opening, closing, cleaning, and staffing the community center, answering phones, making referrals, and other daily and weekly staff tasks


    CORE COMPETENCIES: 

    • Master’s degree in Social Work, Counseling, Education, or Psychology with licensure preferred. Candidates with lived experience of social service engagement and navigating social service systems should also apply.
    • Strong verbal and written communication skills, including strong organizational, conflict resolution, detail and interpersonal skills.
    • Ability to provide and/or receive training on crisis intervention, safety planning, and de-escalation skills, as needed
    • Ability to manage multiple projects with demanding and competing deadlines and the ability to maintain quality work in a diverse, fast paced, stressful and changing environment.
    • Excellent computer skills including word processing, database operations, spreadsheets, and other software systems.
    • Demonstrated ability to work effectively with people of diverse backgrounds including age, ability, gender identity & expression, sexual orientation, education, and ethnicity, in a multicultural environment.
    • Knowledgeable of black and brown queer and transgender issues in the Mid-South, including HIV, homelessness, over-incarceration, sex work, family conflict, and other experiences.
    • Beliefs that lesbian, gay, bisexual, gender non confirming, non-binary, and transgender individuals should live in a world where everyone has equal rights, and is safe, respected, and celebrated.


    The schedule for this position may require some night and weekend hours. This position is primarily on-site with flexibility to work remotely per direction of your supervisor. LGBTQ+ people of color with working knowledge of the South are especially encouraged to apply.
